Depends on how close you are with him. I'd talk to him first then send him your letter of resignation.

If your main communication is via email, email the resignation and offer to speak if he wishes. I doubt he will want to talk unless there is a counter offer.

Try for face to face. If he cannot do it in a couple of days, a phone call with a signed two week notice in the mail is the best way to go in my humble opinion.

I would do it the opposite way. Call and let him know and ask him if he would like an email as well. Might make it easier to forward to HR

I understand...same way at my company. I would try to be a little more formal with a resignation, though. Believe me, your current boss will have an impact on your career for a good while after you leave...may as well have him on your side.
