Yes, Williams made a broader statement that any injury which caused a player to miss several plays was called a cart-off. Then again he also claimed Vilma offered the 10,000 bounty.

None of the other articles I've seen have used direct testimony. So maybe you are right the "cart-off" is just for knocking the wind out of someone and the media and the NFL just ran with it.
