I am assuming that Leo was back in "reality" after being reunited with his kids and the totum about to fall at the very end.

Well I don't think Nolan wanted us to know. It looked like it was about to fall but we don't really know. Very very very good ending, though.

After he went to save Saito from limbo, wasn't their deal that Cobb could return to the United States? That makes the end scene even more "real" to me.

Answers are there. What does he tell Juno Paige right off the bat in dream world. If you start off in the middle of something and u got no clue how u got there it's a dream. Remember how the movie started? Right in the middle with no clue how Leo got there.
Remember how they tricked Crane into thinking he remembered being Kidnapped in the van during the 2nd level? That's a metaphor for the whole movie.

He was performing Inception on himself, maybe subconsciously. He needed to make himself think it was ok to see his kids again and that it was real which he was questioning ever since his wife left.

Yes, he told Cobb he could return. But that doesn't necessarily mean anything. I think Nolan wanted to leave us hanging.

The inception was for Cobb. It was so that Cobb could be with his children despite whatever else was preventing it. A few things:
1) They explained that 1 minute of real time = 1 hour of dream time. That means that 12 minutes of dream time is 1 minute. The flight attendant says “There’s 20 mins until landing” OK- so they were in the 1st layer of dreams for a week? No. Which means the ‘reality’ layer is fake.
2) When he sees his kids at the end, the EXACT shot is used that he kept seeing. Except this time they turn around. His brain was rejecting his kids before, until the inception was complete at the end they couldn’t turn around. This also explains why we see Saito in the beginning. We also see the clock moving backwards in the opening shot, perhaps to show what happened leading up to it?
He had to accept that his wife was dead – she kept trying to get into the layer that he would be with his children – but ideas are like viruses, he knew she was dead so if she was there it would ruin the inception. Thus, we had the scene where he explains to her why he can’t take her with him. She also almost ruins the entire operation, or his subconscious does. She says “How do you know your reality is real? You are running around working for mysterious agencies etc.”

not deep at all. His wife was right, they never woke up. When she left/killed herself he was left alone and came up with the whole leaving the kids behind. Subconsciously he had to make it ok to be with his fake kids.
Clues are scattered through whole movie. Caine tells him in the classroom "come back to reality"
As I previously mentioned the if you start in the middle of something etc etc.
Every scene where the kids are shown is the same exact backwards shot of them, untill after the inception takes place BUT we still see the same shot of them from behind but now that the inception has taken hold in him they turn around.
Nolan tries to trick the viewer like in the plot they to trick Saito and Scarecrow. Whole movie he is dropping this heavy hints that it's possible the world is really Leo's dream still and his wife was right. Right at the end, he gives you the oh no his wife was crazy cause leo inceptioned her. But for the viewer it was just to make us think we were out the dream, when we were always in it. That's what the top is for.
