I've done it twice. All it is is a notebook you fill out for a couple of weeks. You leave one book in each room with a TV and whoever watches TV fills out what they watch in the notebook.

I imagine each notebook represents x number of viewers.
people really need to realize what they are doing when they "fill out a paper"

Electronic metering technology is the heart of the Nielsen ratings process. Two types of meters are used: set meters capture what channel is being tuned, while People Meters go a step further and gather information about who is watching in addition to the channel tuned.

Basically.. sweeps are "it" .. when you see viewership data - it's using sweeps week data 99.99% of the time.
Sweeps take what is already done; the information I gave earlier about viewership data and how it's "gathered"... aka: TiVo, DirectTV, etc., and then adds in these diaries to make it even more accurate.
When you work for a TV company, you are in panic mode leading up to sweeps week - and FWIW, February sweeps are the "King" of them all.. for Networks - July sweeps are equally as important to cable channels. (this is why you heard all the bells and whistles going off about how big viewership had been on TBS for BBT in August of last year)
While it's not 100% accurate, and even I who use to work in the media industry do not fully believe this system is the best of the best.. it's pretty good for what we have. As technology gets better, we will get even better at being able to nail down the viewership data, but for now, I actually truly believe Nielsen is the best we have and will have for awhile.

I know at least 10 people who watch parks and Nielsen doesn't know that. That is just in a 10 mile radius

Coulson wasn't actually a character in the comics until after Avengers came out but I'm sure it'll be something to the effect of Coulson was on an important assignment but they needed him at the forming of the Avengers as well since Tony Stark was so familiar with him so they used a life model decoy in the movie to have him in two places at once.
SHIELD uses the LMD excuse quite often.
