Jimmy showed his cards after the airport heist. Killed damn near everyone.

Sorry for the bump, but the book reads exactly how the movie portrays.
Karen said she saw a guy that worked for Jimmy in the empty store and she always thought the guy did Jimmy’s dirty work. She said it was creepy and she has a bad feeling. She says she didn’t think twice about it until much later. The movie scene is exactly how the book reads.
For what it’s worth, the movie portrays the book almost word for word. The book has Jimmy and Karen’s words as well as the author telling the story. The movie lines are reading the book words practically word for word.
The only glaring differences are: that the movie takes 2-3 people and puts them all into Tommy (Some of the things Tommy did was him, others were other people.) - and the book has more of the timeline that doesn’t make it (Henry goes to the army at 17, etc.). Also, a couple of the actions/narratives in the movie are moved up or combined with another story in the timeline. Nothing big.

Was he? Henry was in jail, not yet out on bail.
He was meeting daily with the authorities who were trying to convince him to rat. He knew Jimmy and Pauly would know about the meetings, so he played it like he was fighting the meetings and telling them to frick off. He was using the meetings to try to get the government to show their cards so he could figure out his options.
Point being, Henry is locked up and they know the government is trying to flip him. So why would Jimmy kill Karen during this time? Henry finds out Karen is gone and he tells the govt he will talk. That’s stupid as frick for Jimmy to do.
The smart plan was to get Henry out on bail so he could be killed. Henry purposely told them he needed to stay in to sober up, so Jimmy wouldn’t bail him out to kill him. And that’s what Jimmy did. As soon as Henry was ready, he had his in-laws bail him out so he could try to collect money and see if he needed to run away or not. Jimmy wanted to meet the day he got out but Henry told him he had a thing with Karen and he would meet the next day for breakfast.
That’s when Jimmy asked him to go kill the confidential informant who ratted on Henry (a small time teenager who was a CI each time he got arrested for selling drugs to save his arse). And Jimmy said Henry was to go with the 2 guys the feds were telling Henry they have on tape talking to Jimmy about killing Henry. Jimmy told him to meet them the following day at a bar he never heard of. Henry drove by and saw the bar was exactly the kind of place Jimmy uses for hits.
Henry wanted to run, not rat, but once the feds heard he was out, they arrested him again that same day (arrested for the airport heist). Henry said they told him he either talked or went to jail for the airport robbery. He couldn’t get back out to run.

Was it a fictional scene for the movie or did it really happen? I'm surprised the question hasn't been answered at some point over the past 30 years.

Jimmy was 100% going to kill Karen. He’d already proven at that point he had no problem tying up loose ends.
