In the books they ship Dorne Gregor Clegane's skull as part of the marriage agreement. There is some debate about whether they used a dwarf skull, but one theory is that they used the real Gregor's head and sewed Joffrey's head on that thing.
Reasons why it could be true: (1) even a misshapen dwarf skull is likely too small to pass as a 7+ foot tall guy with gigantism, (2) Dany has some vision about a knight who open his visor to reveal he doesn't have a head, and (3) (I don't remember if this is in the books) the skin around his eyes is purplish, which is what Joffrey turned due to the poison.
A few reasons why not: (1) I'm not sure how Qyburn got the head given that Cersei never would have allowed it, (2) his head seems way too big for Joffrey, (3) why would you risk pissing off Cersei, just kill some random dude and use his head, (4) that would take the magic done by Qyburn to another level.

But to me that is a problem. Arya doesn't ever truly become no one. She stays Arya throughout her training. She hides Needle. She uses warging to beat the Kindly Man at his own game and tricks him. And she won't truly become No One on the show either. I just thought that was a change in the show that made her faceless man training have less meaning on the show
