Checked the beginning of the pilot and the twins died in 1978. And now I'm going to assume the monster in the basement is the doctor.

I think it's the doctor's kid.
I think the comment he made about he couldn't tell if it was male or female applies to the thing in the basement. Looking at it, you really can't tell what gender it is...err was.
