Tee shots. Im a good putter, a little above average around the greens, average to below average approach shots (depends on the day). Awful off the tee. Its amazing how bad I am sometimes.

Hitting it thin off the fairway with the 56. I am pretty good around the green for up and downs. I know what I am doing, when I straighten my right leg too quickly on the backswing is causing the thin shots (from the video analysis at Man O War).

Honestly, I play my best when I'm distracted and not thinking about my round. The 30 seconds I get in my routine and focused is about all the concentration I use. And really, that's all you need.

that is a cause to a problem...what is the effect?

straightening your right leg, causes your right hip to turn more than needed. having your hip turn too far in the backswing will not allow your left hip to turn completely during the downswing. Which is the reason you are swinging upward instead of decending into the ball. Your body is trying to get out of the way by moving in an upward motion in stead of turning around itself to create speed through impact.

how to fix: Shorten backswing and swing left after impact, making sure you maintain increasing speed through the ball.

Getting through the first three holes waiting for the beers to kick in. Pretty solid playing starts between beers 3 and 4.

ETA: Not trying to be funny either. I think I start to relax and playing more loose.

your swing has always started with an inside takeaway. You need to start taking the club back on a more outside path. If you maintain speed and hip turn through impact, shots should be hit more solid.

Hand movement throwing the club away from your body at the start of the downswing.

Like BP said, if you're taking it inside on the takeaway, the natural tendency is to come over the ball on the downswing. This begins by either casting the club or body movement in transition.

ETA: but I don't know your swing, I'm just speaking in general. May or may not be your problem. I've been hitting all my wedge shots off the toe and checked the lie angle and saw that my clubs were 3 degrees too flat

Consistency

I can play a hole like a PGA tour pro ....then play the next hole like a man that never picked up a club.

Also, I have had several rounds ruined by one blowup hole.
