Number 12 at a course in Oakland, TN called Fair Oaks. The last 3x I've played the course I've been even, -2, and even coming to this hole. Each time I've left it +2, E, +3...and it's an easy hole!!
It's a par 4 that the smart play is to take a 5-6 iron off the tee. It's reachable but driver is extremely risky as there is OB left, and it loops around the back of the green some as well. There are no trees on this hole or around it to block the wind so the lake is usually in play if you go driver off the tee. If you spray it right, the mounds lining the fairway will give a weird lie. A 5-6 off the tee will put you at the 150yd marker, which is what I do every time. The shot from there is all up hill, with the lake in front of the green. I've never not been in the fairway and I can never hit this green. Whatever pitch/chip I leave myself(if I don't leave the approach short and wet) I always hit it to the wrong level of the green.

The freakin 12th hole at Oakbourne CC. I hate that damn hole. It a short par 4 with an elevated tee, and I swear to you the trees on the right have golf ball magnets. I almost never hit the fairway bc it curves slight left from the teebox, so your tee shot has to be very accurate to the left. The trees on the right have massive overhang into the fairway so if you don't hit the left fairway you basically have to punch every approach shot. Not to mention the green is guarded by two huge bunkers on the left and right, and water on the right.
ETA: hole summary from the website:
And that flag is usually tucked behind the right bunker...

Accuracy is the main priority on this tree-lined short par 4. This green is well protected by a large green-side sand trap on the left and water and another sand trap on the right.
