LA LLC Formation question

Do I need to fill out any other forms other than #365 and the initial report 973 to create an LLC in LA?

Do it on GeauxBiz. It had been some time since I'd set one up, but I did two LLC formations last week and also registered a foreign LLC. The process for each was a breeze. Very abnormal for LA.

depending on where the LLC will be located, you may be required to use geauxbiz. they will return your paperwork. just use geauxbiz.

You don't have to have an operating agreement to form the company, but smart people have one, especially if there is more than one member. That's where you provide the details on governance for things like capital calls, tiebreakers, buy-sell agreements, what happens if one member dies or wants to sell his membership to some jerk, etc.

Someone experienced in all the pitfalls of business ownership can help provide for such situations in advance in an OA. That's much better than litigating it later or suddenly finding yourself co-members with your partner's bitch ex-wife.
