re: Ruling on SELA catcher tossing ball to his dugout

When Bregman scored it appeared to be live because the umpire didn't even know where the ball was and hadn't ruled it dead.
After the argument from SLU and the umpires discussed it they ruled it was dead when it went into the dugout. That's why the home plate umpire put his hands in the air and pointed to third after their discussion. He was "officially" awarding the base to Hale even though he had advanced there when everyone thought the play was live.
In the end the official ruling was the same as a wild throw from the outfield going into the third base dugout.

Oh well maybe I thought he was inside the dugout, but in any event if anybody outside of the 9 defensive players touch a live ball it immediately becomes a dead ball and the runners get a base.

This is exactly what I think ended up being the ruling in the end.

A shortstop fields a ball and overthrows first base and hits the first base coach. The ball is live.

A hitter hits a ball down the line that is a fair ball and the ball inadvertently strikes the bullpen catcher in foul territory. I believe that would be a live ball as well.

It's more about the ball going out of the field of play.

In this case the coach is an extension of the dugout, hence the reason they are supposed to be in the dugout. Same as a first or third base coach having to be in their boxes. They are entitle to that spot, but if they are involved with a play outside of that are, they are guilty of interference.

No. The umpires didn't rule it dead at the time. That's why they stood their just watching all of the runners run without throwing their hands in the air. They probably ruled the play dead AFTER the 3 umpires discussed the play.

Once the coach picks up the ball the play is dead.
The other instances where you refer to the ball touching someone other than one of the 9 defensive players are all inadvertent instances. If a coach or fan or ball girl, for example, grabs the ball the play is dead.
In your instance with the Mets, the reason the umps gave 2 bases is that if an OF throws a ball out of play it's a 2 base error as opposed to an INF (which it is a one base error... hence a 2B throws a ground ball into the dugout batter gets 2nd base).
Bregman could have walked home, doesn't matter that he kept running...he was awarded the base.
