just out of curiosity, where did you acquire this dog? Wondering if its genetic as opposed to environmental/diet issues.

You can get a monthly steroid shot for about 25 bucks. Had something similar with my dog.

One thing we did that helped our shep/pit mix was fish oil.
My vet was 100% convinced he had mites, either demadex or whatever the other ones are. He has done scrapes several times and never found the first mite under a microscope.
I try to keep him showered/bathed every 2-3 weeks, and I did it with a medicated shampoo for a while, but now i've swapped to a normal one since I am convinced he doesn't have mites.
The fish oil helped the itching. The bathing is more critical, but we noticed a legitimate drop. Also, we have him on cytopoint shots every few months. THAT helps right away... and from my homework it is much safter than apoquel or whatever it's called.

I'll second, third, or fourth the Cytopoint suggestions. I have a mini Dachshund that would get covered in a rash when he went through fresh cut grass in the summer. Cytopoint injections monthly for about 4 months last summer and it controlled the rash amazingly well. So well that this summer, he didn't need a single injection.

I'd try a new vet. Allergies on an indoor dog is something that should be able to be brought to the point of manageable.
It also sounds like there could be something else at play there as well. May need some steroids like prednisone for the short term.
Been checked for skin mites? Since the coat is so greasy that sounds like a magnet for some kind of mite. Something autoimmune?
Our oldest mini dachshund had some skin conditions when we got him. Prednisone, special shampoo, and some omega 3 fish oil pills squirted on his food fixed him right up.

Honestly, it sounds like you need to take him to a university’s vet school. It is expensive but if you can afford it, that would be the way to go,

Fish oil pills might help the dryness.
