A bone bruise is a modern diagnosis based on MRI imaging. Prior to MRI, we didn't know it existed. Its basically a fracture of the bone without a distinct fracture line. In addition, it is usually a product of a joint that has dislocated or deformed enough to cause a severe impact between its two surfaces. Given that, it takes a significant amount of ligamentous injury to sustain it. For example, pretty much all ACL tears are associated with bone bruising of the femur. We just dont hear much about that part of it, since the player is out for a year anyway.
Bone bruises aren't just from someone hitting your shin. It's a constellation of soft tissue and bone injury that can be painful for many months.
