What’s wrong with my yard?

Is this sod worms? Or some kind fungus?

Centipede yard so I am guessing that is Take All Patch - a fungus. Had the same.
Need to get either Heritage G granule from Site One/Amazon or Fertilome Systemic II Liquid Fungicide.
Retreat after 14 days.
Going forward need to use as a preventative in February before that crap really gets started.

Is it always that wet in that area or is that something new?
If so, it could be a broken water line if it doesn't stink or if it does, a busted sewer line.
Or, just a real low spot in the yard holding water and causing root rot in the grass.

Is a little low right there. This was right after rain last night. It’s not a broken water line.
If it’s root rot, Would just adding some topsoil help?
I may also treat for Take All Patch?

Welcome to my world. I’d start with fixing drainage first and then cut out/re-sod

Only if it doesn't move the low spot to another location. Best to have the ground sloped away from the house to the curb for eliminating low spots. And it don't take much of a fall to make it work, just a few inches overall.

Asoxystrobin is one of the better fungicides. You can purchase that as Scott’s Disease X, think I’ve seen it at the orange box store.
Bad thing about centipede it takes so long to fill in.

It could be take all and it could be large patch. Certainly seems to be killing everything which would be take all. However take all has irregular borders and more yellowing (chlorotic) of the lawn. The circle pattern looks like large patch but large patch doesn’t effect the root system. You might have large patch combined with a suffocated root system due to the water.
Scott’s Disease X in combination with peat moss should get you on the right track. If the area retains water then raise the area or this will continue to happen.

Where do I get peat moss? Does this come in bags from a nursery or box store?

Home Depot, Lowe’s, etc its $12 for 3 cubic feet. 1/8-1/4 inch layer is all you need.
