Is My Knee Area Pain ITB Syndrome?

I hike on a regular basis in the north georgia mountains with a pack(either kid or gear), usually anywhere from 30lbs to 60lbs. Last Saturday, I did 17 miles. Towards the end I was having trouble with the technical declines and was dramatically slowing my pace. Uphills were no problem, nor the graded declines. the two "bands" on the back of each knee were hurting pretty bad. I had the usual after hike pains from carrying a pack that day into the night and keeping my legs straight, at night in bed, caused even more discomfort. Then the next day most pains went away as usual.

I am trying to increase my pace, so I have been running and doing a quite a bit of legwork. Did a leg day monday morning and tuesday was back to running. I was fine for about a 1/4 mile then I couldnt go 50 yards without severe pain in those bands, mostly the outer one. now just simple walking a little bit is causing a flare up. It is localize right at the knee area and does not extend upward.

It sounds to me like you've just got a case of tendonitis. You have hamstring tendons in the back of your knee that connect your hamstrings to your tibia bones. Sounds like you've got some inflammation, likely due to your increased work load.

Your IT Band runs down your side from your pelvis to your shin. Usually results in pain to the outside of the knee or hip. Doesn't sound an ITB issue to me.

A few days rest would probably do you good.
