Not to alarm you further, but if I (almost 46 male) am any indication, then your numbers are off the charts and you definitely need immediate intervention. If your doctor doesn't have dietary solutions that involve cutting ALL fructose and alcohol for the foreseeable future then please get a new doctor.

My AST fluctuates between 10 and 29
ALT: 9 to 23
Bilirubin: 0.3 to 0.6

This was during periods ranging from complete teetotaling, to moderate drinking, to a summer of fun, and some during chemotherapy. Everyone is different, but we're talking decimal places here.

Sorry, I just saw this part and re not drinking.

The obvious culprit is the smoothies. Totally cut out all fructose while you work out the other factors.

I found out when I was in my mid thirties I was born with Chronic Hepatitis B..My virus and viral counts are under Control thanks to Baraclude. It a once a day pill with No side effects.
Interferon on the other hand was a real Bitch for a year and a Half. I still have an enlarged fatty liver and I get a abdomen ultrasound twice a year looking fir cancer along with complete liver panel blood work.
Iwas told a smartman would touch alcohol again.

In terms of health, I have no idea. In terms of quality, cast iron is the shite. Maintain it, and it provides a unique quality to food. Don’t ask me why. It’s an ancient art.

That's a very robust elevation in AST/ALT, much more so than we normally see with just standard fatty liver/NAFLD. The LFTs in these cases range from normal to 3-5x ULN, but its rarely for the AST or ALT to be above 150 or so if this is the only process in play. Also, it generally doesn't elevate your bilirubin. The same goes for chronic infectious hepatitis and hemachromatosis in that your LFTs generally aren't that high.

I'd be worried about autoimmune hepatitis and primary sclerosing cholangitis (was your alkaline phosphatase normal?) with those numbers combined with an elevated bilirubin in someone who doesn't drink and has no other obvious reasons to have liver disease. Those numbers reflect a substantial insult to your liver and I'll be honest, you need to see a hepatologist ASAP.

His AST and ALT are TEN TIMES the upper limit of normal, and his bilirubin is more than two times the ULN. That is NOT fatty liver disease, I cannot stress this enough. He could have fatty liver disease as well but that is not why his numbers look like this at present.

He either took something that induced an acute drug or substance related hepatitis prior to getting those labs drawn, or he has a significant underlying inflammatory liver condition that is driving those numbers. He needs the labs repeated in relatively short order and should see a liver specialist.
