I wouldn't mind a more solo, Man With No Name type story, but Rockstar does supporting characters so well that I think it takes away from their strength a bit. Sure you can get people you run into and cooperate with through the game, but I think getting the audience to care about those characters is tougher than getting them to buy into characters in your gang.
Theres just so much they could do history wise. You could start out east in the Civil War and end up going AWOL or something. The Mexican-American War, getting caught up with the Comanche's, like someone else mentioned the Gold Rush.
It could probably be its own game, but I think it would be cool if you moved your family west or at least went west with a group of pioneers and could choose different routes that impacted your experience. Make it sort of the "tutorial section" like they had in the first game when you're snowed in, where you could spend as little or as much time as you want.

The core and meter approach to eating, drinking, elixirs, etc. needs to go away.
