You don't pick up any heart containers or heart pieces and finish the game with 3 hearts and no game over deaths.
It's a nice twist to spice up games you've beaten 1000 times.

Which Zelda? Any of them? I could probably manage Majoras and OoT honestly, I have those games memorized

Yeah it can be any of them. I’m in the Forest Temple in OoT and it hasn’t been tough to get here. It only took me like 2.5 hours because I’m working though the game pretty quickly without worrying about side quests.

So, if Gannon kills you, you have to start over to finish the challenge? That sounds miserable.

Those have been done to death.
A real good one is finish The Legend of Zelda: Breath of the Wild without switching weapons.

I could probably do it with LTTP but don't you have to grab the heart after each dungeon to advance?

No, the heart container is an optional pick up.

I did this with OoT and Link to the Past. Link to the past was infinitely harder. The Big Goron Sword is so damn OP.
