100% luck in any leagues with 10 teams or less.
While I'll agree that more strategy and skill is involved with larger leagues, no league is 100% luck. You and I could hold a 2 team draft and luck would be a huge factor, but one of us could also have a better strategy for picking in that league.

Primarily luck.
In one league, I was the leading scorer, and finished 7-6. My PA was about 10% higher than the next closest guy. I built the best team (obviously), but many games it didn't matter. I was a 2 point victory from not making the playoffs. I had a top 10 QB, the top two RBs, 2 top 20 WRs, a top 5 TE, the #1 K, and a top 10 D. All but one of my losses were the most points scored by that team all year. 4 of the 6 losses, I would have defeated any team in the league EXCEPT the one I played.
In my other league, I also finished 7-6. My team wasn't nearly as dominant (thanks Ray Rice), but still very solid for a 16 team league. I lost one game by .2 points (somebody got a bad spot, or didn't fall forward), so it came down to points. I missed the playoffs by 10 points. Eli (or Mark Sanchez) both had their lowest output of the season (13 points below average) last week, against the #29 and #32 passing defenses. I fretted all week about which one to start. In the end it wouldn't have made a shite of difference.
What can you do?

Scored the most points and had the most scored against me. I was consistent the whole year. My team ended up 6-6 with a 6th seed in the playoffs, having to play the second highest point scorer in rd one. (12 team league, 6 teams in playoffs)

You will have to post a picture of your standings for me to believe that you scored the most points and finished 6th. I'm not saying its not possible but extremely rare

I had the most points in my league, leading one guy by a few, and everyone else by at least 100. I won a game 143-141 three weeks ago.
If that game goes the other way, I finish 6-7, miss the playoffs, and end in a 5 way tie for 6th place.
