The surgeon thing probably has something to do with open wounds being way more susceptible to droplets causing problems then when there are no open wounds present.
Also comparing a completely sterile environment of an OR to a grocery store seems a little off.

That was my thoughts as well. Imagine a surgeon with someone's chest cavity opened up, no mask, and accidentally coughs right into the patient's internals.

I was making a point about the study claiming that wearing masks for an extended period of time is detrimental to the own person’s health which is clearly not true
