Question for Plumbing Experts RE: Sewer Cleanouts

This morning I was cleaning out my flower beds in the front of my house. I’ve always wondered about this so I figured I would post it here. What the purpose of the third cap the one on the very left? It’s just a pressure fitted cap on a 4” PVC pipe.
I know the two cleanouts are this variety...
But what is the purpose of the capped PVC on the left? Slab house in Huntsville, AL

Don't know what the purpose is for the one to the left. I wonder why the plumber didn't use a two way clean out Tee instead of having two clean outs?

I’d put money on the fact that he simply didn’t have it on his truck and didn’t want to make another trip to the supply shop.

The third may be a storm clean out. Are your downspouts tied into a storm drain?

No my gutter downspouts are just the traditional ones that terminate at the base of the slab and I have 6ft extenders on them.
