You are being idealistic and this study was practical. Idealism completely ignores human nature.
For instance, your idealism ignores the shocking amount of people on Medicaid who would rather show up at an ER at taxpayer expense rather than buying aspirin at Walgreens

No, I am reading what you posted(and adding that it is in line with other literature on the topic). It does not support the earlier assertion you made. You are misreading the literature you are posting.
Preventive care is largely substantiated by the research to be cost effective but not providing meaningful cost savings except in a few areas like vaccines. It's cost effectiveness though is a good reason to invest in it and it is why private insurers have and continue to do so.
