They have health ministries, but many aren't for the things we think of in terms of health care.
Some catastrophic, managing epidemics, country- and region-specific diseases like malaria, etc.
Many of them do the vast majority of their health care through religious organizations or private systems.

Well most third world countries do most of their things through charity orgs and outside orgs because the people are too poor for taxes to be collected

I don't think this is wrong, but I do question your interpretation.
Namely because of what this study(and others like it) do and do not actually show.
They do show that if you are focusing on bending the cost curve, there is only a very narrow sect of preventive care that is effective at this(vaccines, low-dose aspirin etc.) but there is a lot of preventive care that is cost effective(meaning that while they don't save money and only break even, they provide a lot of large additional benefits I.E. improve people's lives). A lot of preventive care is cost effective and it is in part why many private insurers still encourage it. They don't show that just treating things as they pop up is cheaper either. They show that compared to the baseline(what we currently do) current or additional preventive measures will not bend the cost curve downward much, if at all.
The other thing not focused on is the secondary benefits of a healthier populace. There are also a lot of macro and micro studies about the productivity increases from improving and maintaining a workforce's health level.

You are being idealistic and this study was practical. Idealism completely ignores human nature.
For instance, your idealism ignores the shocking amount of people on Medicaid who would rather show up at an ER at taxpayer expense rather than buying aspirin at Walgreens

The perverted thing is, Obamacare punished working people with employer plans in their pocketbook.
Then they try to be responsible and take part in wellness programs so their fleecing can go a little further with fewer claims

No, I am reading what you posted(and adding that it is in line with other literature on the topic). It does not support the earlier assertion you made. You are misreading the literature you are posting.
Preventive care is largely substantiated by the research to be cost effective but not providing meaningful cost savings except in a few areas like vaccines. It's cost effectiveness though is a good reason to invest in it and it is why private insurers have and continue to do so.

If a healthy workforce is your anchoring point, not sure how walling out anyone not currently employed is an optimal path to achieving that?
