That episode may have been the best one of the series. Her being the fake daughter of Culverton surprised me, but when she revealed that flower behind her ear, that just blew my mind. Now granted, having it be Sherlock's sister was a little goofy, but I don't know if Doyle ever wrote Sherlock with a sister. But the way that the episode played out with Sherlock putting himself in a position to risk being killed in order to get Watson to snap out of his rut, only for it to turn out that his sister was the one manipulating everything was well done.
Although I do wonder how Sherlock Holmes, the most observant character ever created, wouldn't have recognized that Faith was actually his sister, even in his drugged out state.

I didn't see the ultimate twist, but for some reason her presence early in the episode made me think she would play a role later on. I thought she was.... (TWIST TWIST)
SPOILERS...
I thought she might originally be the one who tipped off Smith, or might be his daughter in disguise? Obviously that didn't work out.
