In addition to all of the stuff they told you to do in this thread, send a letter to his firm requesting his E&O insurance policy. If he is ineligible to practice, I think the bar will appoint someone to respond to inquiries made to his firm.

His firm is an empty office, should I send to the disciplinary committee?

Theoretically a good idea. However, I've seen several of these cases, and at one of these guys having a "firm" and at one of these guys having insurance. But I guess you have to check.
