Is there an app or website that I can just type in the channels I want and it spits out the best streaming hardware and/or streaming services that fits my needs?

I'm not aware of any app like that. Most IPTV services offer basically everything. Picture getting the biggest package with DirecTV with all the add-ons and that's an IPTV provider.
I did a search and check out lemotv.com. They offer a free 36 hour trial and if you like it, then a 6 month subscription is less than $30. I'd recommend getting tivimate as well. It has a great TV guide. It's free but there's some cool things if you get a year subscription. I believe it's $5.
If you, or anyone needs assistance getting set up. I'd be more than willing to chat privately.

For IPTV, most will have any channel you think of
