ITT: Most Dreaded Things in Video Games

Any fan of racing games in general knows that the Blue Shell of Death on the 3rd lap strikes fear into anyone in 1st place.

made it through 3/4 of elder scrolls oblivion with only my auto save as backup.
ate a poison apple and walked through a door where it auto saved
other side of the door I was dying and couldn't heal myself
my auto save backup was after I had ate the apple
never finished oblivion

or an auto save feature saving when you have 10% health in a crowded room full of enemies and you have no ammo.

That almost ruined one of my Metro playthroughs. Started a mission, ran out of filters, couldn't find any in time, died, loaded right back into the filterless hell.
I also hate long drawn-out cut scenes that suddenly present you with stupid QTEs that you inevitably miss, so you die and get to start the cut scene over again.

Keep the Moron alive quests.

-Lag in the form of teleporting in online games.
-Bad camera angles in platformers (Tic Toc Clock in SM64 was really bad with this...)
-Falling through platforms in old side-scrolling games that you cleary should have landed on (Castlevania I'm looking at you here)

Games that have long drawn out death or Game Over screens. Make restarting a long process. You just want to try again and get it right as quick as possible and instead they're rubbing it in that you screwed up. Gotta wait for the villain to laugh at you, the game to tell you you suck, wait for the main menu, reload your savefile...
