Would it still be "Judge Smith"

Call him by his first name, and act like a boss when you do it.

In what context/setting are you meeting him ?

In writing it should be "The Honorable _______". Orally it should be "Judge ________".

You always address a person as the highest title they have achieved.

Right. I work at a restaraunt and I refuse to call my boss chef. Even our head. Same with professors. Them frickers ain't doctors.

So when playing golf with a judge this statement is actually possible. It's an honor your Honor your honors.

If just call him "Judge" like a nickname.

Formally, and if I'm trying to be proper, yes, Judge kind of sticks with a person.
If the guy is over at my home for dinner, I'm calling him his first name, or Mr. ____
