Question about the Smoke Monster (lost)

In short the Smoke Monster was a convenient tool that writers could use to get themselves out of any ridiculous plot point that they needed to.

What I want to know is how the hell did Christian appear to Jack in the hospital (after the smoke alarm goes off) and how he appeared to Michael when he said "You can go now". The Man in Black can't leave the island. If he can appear on the boat, why the hell can't he ride the boat to the mainland?
