No, there were no Lannisters present at the Tower of Joy when she died. The only possibilities for her death are: a member of the Kingsguard killed her before being cut down by Ned and Howland Reed, she died of some disease that finally took her right as Ned finished his fight with the Kingsguard, or she died in child berth right about the time Ned finished disposing the Kingsguard.

However she died, she lived just long enough for Ned to hear her final words (after killing the Kingsguard), "Promise me, Ned"

Ned also remembers her having blue roses in her hands. A blue rose is what Rheagar gave Lyanna when he named her the most beautiful girl at the tourny.

Just a guess but I'm thinking Lyanna died either of childbirth (though I'm not sure how the time line of Jon's birth plays out there) or of some type of disease.

The people with Ned wouldn't have wanted to kill her of course. And I'm not thinking the members of the King's guard would, though I guess that's a least a possiblity - maybe under orders sent by the mad king in case the tower was found/breached. Even then, I think they would have done it more quickly and decisively for several reasons.

I don't think this is necessarily the case. I think there was some time that passed by before she died. Ned's memories fit perfectly with someone that is hiding a huge secret. Even in his thoughts there is little information from his memory of the Tower of Joy. If he had to hide this from the entire realm, including the people he loved, he would likely have to train his mind to think of only the events he could mention. Thus you get a recollection of events that seem like they are all happening in quick secession.
