And how do games that are released in series, year after year, continue to have the same problems the first few weeks? How is this not clean on release? Is it always inevitable?

Depends on the problems...party and connection issues are server-related and can't be stress-tested until release.

Anything network-related is a roll of the dice IMO.

You'd be amazed and probably shocked how little the right hand talks with the left hand in regards to fixing common platform-wide issues.

The "devs" are working new shite by the time a games released, so the ones working the "fixes" are probably a whole new team of people. The devs probably never know what was wrong when it went live.
