Tragedy in Chinácota: Woman Dies After Accident on Water Slide
A 28-year-old woman, identified as Yiris Cristel García, originally from Tibú, Norte de Santander, lost her life in Chinácota, Norte de Santander, after sliding down a water slide at a tourist attraction.
According to initial reports, Yiris climbed onto a tire that was to slide her down to the lower part of the slide while an instructor indicated the correct way to position herself. At the moment of descent, screams were heard, and it is presumed that the woman tripped or suffered a blow that caused her fatal fall.
Authorities have launched investigations to clarify the facts, focusing especially on whether the attraction had the appropriate safety measures in place to ensure the integrity of visitors.

Judging by the video I'm guessing that on that hard left turn the tube went left and she kept going straight over the edge.
