Looking for recommendations for a spa hotel in Tuscany.

I wish I could help you.

Not a "spa hotel" but twice I stayed in a wonderful villa in the Tuscan hills about 20 miles south of Florence, but I was sorry to see that the owners have recently sold it to someone who is using the compound as a private residence.

Look at Il Borro. Owned by the Ferruccio Ferragamo family,

Il Borro

They have a world class spa, the food is outstanding, the wine is awesome and the medival town is first class. About an hour south of Florence right in the heart of Tuscany. Near Sienna and Volterra. Both great towns. You can rent a car and drive around the countryside as well.

La Foce outside of Montepulciano is also a great spot. I don't believe they have a spa, but their villas are great and their garden is spectacular.

Banfi Winery located inside the Castle of Poggio alle Mura just outside of Montalcino. I think they have spa, but not sure. Their wine and food are very high-end and the location is amazing.

Villa petriolo is nice. Brand new spa, good restaurants on site.
