Admittedly dumbass question about internet service

Telephone company laid fiber down the road in front of our hunting cabin.

Say I want to get that service (internet) to the cabin.

Who is responsible for getting that fiber from the road to the cabin? Do they install it completely and recoup that cost over time as I pay for the service? Or do they do it and charge me?

How does that work?

I'm asking here because I never can remember to call those folks while they are open.
