To be fair, it had been 200 years and they may not have had as much in-person interaction with her then and she did look pretty different in the Ghoul's flashback. Even he was a little confused when he saw her picture on the wanted poster. Hank did recognize her though when he got up close with her.

Like someone mentioned earlier, she wasn't a part of Vault-Tec. Her company got bought out, and there were so many that were frozen that who's to say he ever actually saw her. Plus, if she figured out a way to sneak herself into that program, another reason he wouldn't have recognized her.
