Eddie Haskell set the bar in that Pleasantville era of TV. He was about as bad as kids got on 1950's TV.
From when I was a teenager, two that stick out:
Pam on One Day at a Time. The one that's not Valerie Bertinelli.
Gloria from All in the Family. I think she was supposed to be a teenager when it started. She was in college, but he was a graduate student already. Probably one of the reasons Archie hated him so much. That and his feet sticking.

More modern times finds me absolutely hating A.J. Soprano. I can't think of many scenes where I didn't want to reach through the TV and choke that little bastard out.
Then a bit closer to now------Joffrey from "GOT" really took it over the top as far as being annoying.
