Did anything ever come out explaining this incident? Like an official diagnosis?
She caught her true reflection in the van windows and it dropped her like a stone. The mirrors of the Clinton Foundation don’t work that way.

Don't you have to win a primary to be the nominee
No. Not as I think you mean. There is no requirement for either party’s nominee to win enough delegates, by popular vote, to secure the nomination. The requirement is that a majority of delegates vote for them. That’s it.
It’s only in recent times that the nominee is always the top vote getter in the various primaries and caucuses. Abe Lincoln, for example, only had 102 delegates after the first convention vote. He won the nomination on the fourth ballot with a Tigerdropping-auspicious 350 votes.

The Clinton's still have much influence within the Democratic party. Biden will not be the Dem candidate. Obviously, he(Jill) are refusing to step aside. Willingly or unwillingly, Biden will step aside.

I think a lot of creative Sec. of States' might find reason to remove her from any ballot.....isn't that the way we are playing the game now?

Imagine both Hillary and Michelle entering the race. How would Hillary(especially) tactically go after Michelle without pissing off the black vote? That's a hornets nest she'd want no part of.

She's too lazy to get involved. Now Michael may be a different story ...
