Obviously not done correctly if line was still pressurized. To do proper LOTO you would go back further in order to vent the pressure off safely before working on the line.

Its not an OSHA plant for one, if the pipe was plugged, depending on where the bleeder is, you could correctly loto and verify no energy at the bleeder but still have this happen
