It’s really not much different than any American city. Uber, blacklane, have hotel call a cab

Having just arrived home from Rome last night, if you are planning on driving...be prepared. It's like one big game of chicken. No lanes, if four cars can fit across the road, they will. Also, motorcycles go anywhere they want. Stop signs/lights are suggestions. There is also no "parking lots." All street parking, so be prepared to look for a spot for awhile and walk. My husband is an aggressive driver so he was ok, but it was scary, lol.

I would never drive in Rome. We were in a taxi and came up to a, I kid you not, 8 way intersection.
Naples is worse. Taxi rides there are literally on two wheels.

You don't want a car in Rome. We got one in Rome one time on our second to last day there because we wanted to take a day trip to Tivolt, Hadrian's Villa and the Gardens of Villa d'Este (which is beautiful, by the way) and we were driving to Tuscany the next day. Traffic was bad enough but the worst part was the parking. Our hotel did not have a parking garage and downtown parking lots are very expensive in the unlikely event you can find one at all. The hotel would have valet parked the car for an exhorbidant price, but we parked at the train station and caught a cab to and from the hotel. A real pain.

We arrive in Milan, have a rental to lake como then, to, in, and around Tuscany also by car. Dropping it in Florence and training to Rome for final three days. Hopefully will be enjoyable driving from como to panzano. Not driving in Florence or Rome. Thoughts or adjustments to this plan welcome.

A car is necessary in the Tuscan countryside, and you will be fine with it.
