I really don't know how this happens. When my two were little I would bring them to my in-laws every day. Some days when my wife would stay home and keep them, I would still drive to my in-laws before I realized they were at home with her. Even on days I would drop them off when I would get to work I turn around and look in the back seat. It happens, but I just can't understand how it happens. From the time I would get in my vehicle to the time I got out, I was constantly mindful of them being in the back seat.

This is exactly how it happens. We often go on autopilot based on our routine. These situations seem to almost always happen when the parent who isn’t in the routine of taking the child to daycare has that responsibility on the tragic day. They go on auto pilot and miss the trip to the daycare. There are several good ideas in this thread to create a physical reminder that can prevent this.

My wife and I don't really have many situations where this is likely (we don't do daycare), but we've nonetheless started putting our bag/purse in the back every day so we have to open the back to get it out at work.
