It was very rarely enforced. It was a joke in the 90s and 2000s.

I saw an old clip on YouTube where Elway is playing in Arrowhead and he keeps backing off the ball because of the crowd and the refs have to give the crowd a warning, crazy.

Elway was pissed it was loud.

I never seen any penalties issued but it was far from a joke. Many times at LSU home games in the 80's when it started the QB would back away from center claiming he couldn't hear and ask for help from the ref. They would make a threat of a penalty and it only get louder.
