I grew up 15 min outside of Moundsville towards the PA border. Are you talking about Dicarlos? It always has mixed reviews. Some people love it and some don't like the toppings added after cooking the crust and sauce.

it was not square
it WAS deep dish, and one piece seemed like it weighted two pounds, but of course I'm exaggerating a little
that name sounds right though...

Only place I can think of was Pizza Outlet that got bought out by a local western PA chain Vocellis which is very good.
