Anyone have any first hand experience? Wife and I are planning on going fall 2023.
We like smaller towns and hiking/outdoors.
Taormina, and the ancient ruins are on our list for certain.

Palermo has the best street food and Byzantine Mosaics/Art/Architecture etc
Catania has the best restaurants
Siracusa has a great old town and home of Archimedes
Small towns: Cefalu (Beach, Cathedral and mountain), Agrigento (Valley of the Temples and Capital of Arancini), Taormina (walk and greek theater), Trapani (salt fields), Monreale (Cathedral and walk around)

We took ferry from Southern Italy to Messina and then drove to Taormina. Loved the town so we decided to stay the night. Great walkable city, fun times.

I grew up going to Sicily in the summer. Great recommendation. Cefalu is a fantastic town.
I'd rent a car and do Mt. Etna. Pretty cool to drive up.
Caltagirone is another small Sicilian town that is great. I'm partial also to the southern end of the peninsula as that is where my family is from. Marina Di Ragusa is a great Beach Town.
As with all places in Italy, you really can't go wrong, even more so in Sicily due to the smaller percentage of tourists. You'll enjoy yourself regardless.

We stayed at Hotel Villa Schuler in Taormina. Would go back!

Yes--adore Sicily in all of its complications. Having a car is a must--it is not well-served by public transit, and without a car, you won't get into the deep interior or nooks & crannies.
Are you flying into Palermo or Catania? Planning a driving route depends on whether you've got to start/end in the same place or not.
Agree that Taormina is a gorgeous spot, and Etna is worth seeing up close and from afar. If you're staying in Taormina, try to find a place with a view. If you're economizing, you can stay down in Giardini Naxos for a LOT less and drive up to Taormina for the day/evening. Big public parking garage on the edge of Taormina makes it easy.
A surprise spot we ended up enjoying tremendously was the pilgrimage site of Tindari. It's on a promontory overlooking a beach/lagoon, the site of an ancient Greek town (with excavated ruins) that is also a historic Catholic pilgrimage site with a Black Madonna figure. Can be clogged with tourists in peak season, but it's a pretty spot in the off seasons.
Also consider going to one of the Aeolian islands, if you travel along the northern coast. Vulcano, Panarea, etc or even Stromboli if you're super into volcanoes.
Palermo is fried-street-food heaven. Damn, if I only had some arancini right now. If you're into food markets, the Ballaro market is still full of actual shoppers, not just tourists.

Fried foods, that's me. Cafes and markets in town squares are fantastic for us.
We are thinking of flying into Catania and renting a camper van. Then heading south and looping the entire island. Return to Catania.
We did 17 days through Italy last year in a camper and absolutely loved it.
Did not have any issues getting around and it gave us plenty of flexibility.
Looks like they have many smaller towns to explore, which is what we liked in Italy. Volterra, Orvieto, and Lucca were great.
Will definitely research everyone's suggestions.
I am old school, research, get a map, highlight it with interesting places, and go where we feel.

I've been to Taormina. I saw Eddie Vedder play in the ancient Greek amphitheater there. One of my favorite beach towns in the world. I will be back. Some things you gotta do: see a concert in Teatro Antico if you can time it right, go down to Isola Bella, go up to Castelmola (and hike up to Monte Venere if you're in good shape), eat the fruit from a local fruit truck, and stay in a hotel with a nice pool and view.

I saw the theatre and it looks like it would be an awesome experience. I wonder if they put on a lot of events. It seems like it may just be in the summer time.
Would definitely move my dates around to catch that.
The entire vibe of Italy is amazing, can't wait to go back.
