Little towns in Tuscany that are worth seeing. Stay in Perugia and visit wineries and Assisi, Sienna etc. Was my favorite area in Italy.

When we went my wife packed in a BUNCH of stuff in each location, but we also planned 1 or 2 days in each location to ro-day / chill.
Italy has so much to see and learn. You can miss a lot of the beauty and history being too focused on a packed itinerary.

IMO 4 nights in Venice is about 3 nights too many.

Congratulations on your anniversary . A couple of points echoing, building on what others have already suggested:
Train stations in Milan and Florence and Rome are crowded — allow extra time. Book refundable business class seats if you can. There are two major train lines in Italy. I have only used Trenitalia, their app works great.
A sturdy airline approved carry-on is a must. Pack light. You’ll need to lift your bags into tight overhead racks, even in business class. Not too many people will help you. You do not want to schlep luggage. If you have older people traveling with you, consider taking a small anti slip bath mat.
Scan important docs to your phone, know your credit card PINs. Bring basic first aid items (Tylenol, antacids, Band-Aids, earplugs).
Stick to wrinkle-resistant, reversible clothes you can wash overnight. Tip housekeeping; build rapport with staff and management.
Day trips are fun, but even short ones can take most of the day. There are some good YouTube vids on italy train travel. You will get a better idea on how to read the departures and arrivals. For those must sees in Florence or Rome be sure to investigate whether you have to book in advance and book via the “official“ sites.
Also, learn some basic Italian phrases. It’ll be much appreciated.
