Usually just a popularity contest. UEFA TOTY always heavy on Barca and RM. Granted, there is a good reason for this but not sure it is always justified for those two teams to routinely lock up 8-9 of the top 11 in the world.
Edit: That was not quite the case this year but had been for the 3-4 years prior.
