Well then more specifically, how are leaders really chosen here in the US? Do we actually elect them or are they chosen ahead of time? Who chooses them? How do they decide? What about other countries? Is it one group that runs the entire planet or are there several powerful entities?

Who are #1&2 picks to run in 2020?
I don't own a crystal ball, but IMO the establishment will put Trump back in office if he's physically able. He's done a magnificent job for them so far, I don't see them making a change.

So it really doesn't matter who the other picks are to run in 2020.

Think about it, if you owned a corporation, would you decide who runs it or would you allow perfect strangers to vote on that decision?

That's a good question. I don't believe there is one single entity controlling everything. But it's pretty simple: if I have a billion dollars and you have one dollar, who's in control?

I used the corporation as an analogy. But, yes, in our Fascist rule, corporations do make policy and the major ones do have some control over government. The Military Industrial Complex springs to mind, as does the International Banking Cabal.

Different wall, and was part of a negotiated deal. Sometimes you get stupid things in exchange for needed things, because Republicans won't vote for the needed without the stupid.
