Yeah Internal Medicine and especially Family Medicine always have lots of unfilled spots because those specialities aren’t as desirable and because of a lot of those unfilled programs are in rural areas and/or have accumulated bad reputations through the years.
Most US grads do not want to go to smaller places like Florence, Alabama, Charleston, WV, Searcy, AR, etc.
Regarding bad reputations, systems like the HCA hospitals are largely looked down upon and most of them have a hard time getting many US grads to apply to them even if they’re in bigger cities. And so they’re filled with IMGs.
When I was applying the data is available to the applicants so I remember seeing some of these places having like 70-80% of their applicants being these IMGs. And of the 20-30% of the US grads, the majority of them would rank these places near the bottom of their rank lists

Some (not most) of those unmatched US grads are people who applied badly. I remember in my class I had some friends who applied to a pretty competitive specialty (basically no IMGs here) without having a backup IM or FM programs. They ended up SOAPing into IM or FM programs but they took a dumb risk for no reason
