It could be a part of wire/transmitter radio system that's linked to an earpiece. But that seems like it would be more noticeable.

There are newer iv bags that are the size of a small ball. They infuse not by gravity but by vacuum of the ball. The ball deflates infusing the medicine. Not saying that’s what that is but technology has advanced in that regard.
