These are probably some old fricks, who would make you call them Mr. if they weren't a Dr. anyway.

I don't know a single doctor that wants to be referred to as "Dr." outside of their profession, I'm sure they exist but all of the doctors that I know are younger and that's not a thing that the younger docs typically do.
