I believe my car is having some transmission issues (grinding and sometimes the gear shifting is pretty rough). The dealer can't replicate the issue b/c it's sporadic. I'm worried that something will happen after the warranty is expired and I'll be left holding the bag. Is there anything I can do to protect myself from any future break down? Right now I'm thinking my options are to either get rid of the car or buy an extended warranty (which i never do).

keep bringing it there until they have a representative come in and tell them to replace the transmission.

Extended warranty is a scam. Just leave it running with the doors unlocked while you go grocery shopping

Now that you told them about the problem it probably wont be covered in an extended warranty.

Year/Make/Model?

Join a marque-specific car board and see if others are having this issue.

Use Google to see if the manufacturer has released any TSBs related to your issue.

Take it to another dealer or mechanic authorized to work on whatever you have. Keep the documentation of your visits.

Make sure you are not just hearing things.

This is great advice. Do it with all my vehicles.

Start hard stepping the gas pedal right before you feel it shifting and completely let off gas after it shifts. Absolutely terrible for your transmission and will probably break something before your warranty expires

Figure out what makes it happen, and take the tech for a ride

I disagree on the "do not buy an extended warranty" advice.

It really depends on the make/model of car or truck.
While I understand that generally extended warranties are not a good buy, a lot of factors should play into that decision.

For example:

Are you going to keep the vehicle for a long time?

Do you annually put so many miles on the car that you will quickly exceed the basic warranty?

Does the vehicle have a negative or positive long-term repair history?

Is it the first year of a new engine/drivetrain?

Is it a diesel truck, high-end sports car, or unique vehicle?

Next time it starts acting up video it, it might pick up the grinding and will definitely pick up if it’s shifting bad. I’ve done this a few times and it helped the service technician tremendously.

Do like my whore sister and stop paying the person that loaned you the money for the car.

What Brand?

Coworker called Ford corporate after experiencing the same thing about 5k miles before his warranty expired. They offered him a 36k extended warranty at no cost.

ETA: It’s worth the phone call if your local dealer can’t help you further.

You're complaint is on record so that should help.
Only problem is one of the stoner mechanics will plant meth in your car somewhere and alert the police.

Literally all of that shite is baked into the warranty.

The people that sell those things are way smarter than you and wouldn't sell them if they couldn't make money.

Cars always stop making the noise when you take them to the mechanic. You have to get a passenger to film the issue.
